Identity and Statement of Educational Purpose Verification

If this item is requested on your account, the government has selected you to provide the following:

• An unexpired valid government-issued photo ID, such as a driver’s license, other state-issued ID, or a passport (must be original).
• Completed Identity and Statement of Educational Purpose Verification form (provided during appointment).

This must be completed in person in the Office of Financial Aid. You can schedule an appointment here. You must complete this requirement within 60 days from the request on your account.
