1098T Statement Request Form

If you would like to request a statement for information regarding payments you made to Aurora University between January 1, 2007 and December 31, 2007, please use the statement request form below.

If you are a current student, you can access your electronic statement through the online Student Accounts Center. This can be accessed through the WebAdvisor "Make a Payment/View e-Bill" option.

If you are no longer a current student, your statement will be mailed to the permanent address we have on file. If you have moved since you were a student and have not updated your student address, you should first complete a change of address with the Registrar’s Office and then come back to this screen and complete the 1098T Statement Request Form below. We will only mail your statement to the permanent address on file in the Registrar’s Office.

If you did not receive your 1098T form or if you need a duplicate 1098T form, please call 630-844-5625.
