Do I Have Everything Signed?

At the beginning of the academic year, students and parents quickly learn that many campus offices require completed forms. This checklist offers you a quick way to verify you have returned all the necessary documentation. Financial Aid:

• ____ Free Application for Federal Student Aid (FAFSA) Form
• ____ Student Loan Request Form
• ____ Parent Loan for Undergraduate Students (PLUS) Form (if applicable)
• ____ Parent Authorization Form (only for parent)
• ____ Student Authorization Form (enables financial aid to be applied to miscellaneous fees)
• ____ Institutional Verification Worksheet (if requested by Financial Aid)
