Textbook and Software Requisition Form

Use this form to submit textbook requisitions to the AU bookstore and to notify AU Information Technology Services about software you intend to use in the AU computer labs. You may use this form to submit up to 5 items. Additional requests should be submitted on a separate form. Note: This form is designed for use by faculty ordering textbooks for their classes. Students who wish to order textbooks online can do so at the www.aurora.bkstr.com.

Directions: Fill in the form as indicated. Fields with the * asterisk and red labels must be completed (exception: fields in the Book Orders section are not required for unused items). Additional fields may be required depending on answers to the required fields. The system will not accept the form without the required fields completed. When the information is correct, go to the bottom of the form and click the SUBMIT button.

Lab Software Requests
If your course requires that software for use by students be installed in the Computer Labs we need to know in advance. This may include software that accompanies textbooks which you also want to make available in the labs, as well as separate software titles not associated with textbooks. It is not necessary to request standard software already installed in the labs (such as MS Office and SPSS). Installation of course software in the labs is subject to licensing restrictions and compatibility with our lab machines. Information Technology Services will need a copy of the software for testing purposes at least a month before classes begin so that the application can be loaded and tested. Without access to a test copy of the software, ITS staff will not be able to guarantee that the application will be available for students at the start of the term.
